Background

This Supplemental Proof of Claim is filed in compliance with the requirements of 11 U.S.C 501(f)(1) as the Debtor was granted a forbearance under the CARES Act (15 U.S.C. § 9056 or 9057).

This event will NOT appear on the Bankruptcy docket, but will appear on the Claim Register. Please remember to use Official Form 4100S, Supplemental Proof of Claim for CARES Forbearance Claim, when filing this event.

 

Step-by-Step Procedures:

  1. Select Bankruptcy>Claim Actions

  2. Enter case number and click Next

  3. Select Supplemental Proof of Claim for CARES Forbearance Claim (Form 4100S)from event pick list and click Next

  4. Select Party(s) from party pick list or Add/create new party, click Next

  1. The Attorney/Party Association screen may display.  If it does, click here.If not, click Next, and go to step 6

  2. The following reminder will appear, click Next

REMINDER: This event will NOT appear on the Bankruptcy docket, but will appear on the Claim Register. Please remember to use Official Form 4100S, Supplemental Proof of Claim for CARES Forbearance Claim, when filing this event.

  1. The PDF Document selection page will display.

  2. Browse, View, and Attach, click Next

  3. A List of Claims will appear. Select the appropriate claim from the list, and click Next

  4. The Final Text screen will display.  This is your final opportunity to modify the entry before submitting the filing.

  5. Verify the accuracy of the docket text.    If not correctClosedclick bankruptcy to start over/if correct

  6. Click Next to complete the filing process
